Documents required to be submitted before the diploma examination

The following documents must be submitted to the Programme Office responsible for your degree programme no later than seven days prior to the scheduled date of the final degree examination. Contact details for the Programme Office can be found on the following website econ.uj.edu.pl.

Each document should be downloaded, completed using a word processor (all required information must be entered and all relevant options selected within the active fields), and then printed.

Document templates available for download and completion:

  1. [mandatory] Application for the issuance of official copies of documents certifying the completion of higher education studies
  2. [mandatory] Declaration regarding participation or non-participation in support schemes implemented at the Jagiellonian University for projects co-financed by the European Union, such as the “ZintegrUJ” or “Doskonały Uniwersytet” programmes
    1. [optional] Declaration confirming the fulfilment of obligations as a participant in the “ZintegrUJ” Project (Declaration required for participants of courses and classes conducted under the “ZintegrUJ” programme. Further information is available on the website: zintegruj.uj.edu.pl, in the section titled “Declarations for the Diploma Examination”. Please note: the website is available in Polish only)
    2. [optional] Declaration confirming the fulfilment of obligations as a participant in the “Doskonały Uniwersytet” Project (Declaration required for participants of courses and classes conducted as part of the “Doskonały Uniwersytet” (Excellent University) programme. Further information is available on the website: doskonaly.uj.edu.pl, in the section titled “Declaration of compliance with regulatory obligations – concerning point 5 of Order No. 3 of the Rector of the Jagiellonian University dated 11 January 2021”. Please note: the website is available in Polish only)
  3. [optional] Declaration on access to equipment and technology enabling the diploma examination to be conducted in a remote format
  4. [optional] Authorisation for a third party to collect the degree certificate along with its official copies and diploma supplements (If you know you will not be able to collect the certificate in person, you may authorise another individual to do so. For this purpose, a written authorisation must be signed in the presence of a faculty office staff member administering the studies, or before a notary public)
  5. [optional] Information on the planned presence of other persons during the diploma examination – applicable in cases where the examination is conducted using electronic means of communication, and where the presence of other persons results from arrangements with the Jagiellonian University Disability Support Service regarding the adaptation of the diploma examination to the needs of students with disabilities.
